Direct Primary Care Memberships & Payment Structure

Gloria Gates CARE offers Direct Primary Care (DPC) memberships for a flat monthly fee. This membership provides access to enhanced care coordination and services as outlined in the applicable membership agreement.

Medical care provided by licensed healthcare providers may be billed to and paid for by the patient’s health insurance, when applicable. The DPC membership fee is separate from insurance-covered medical services and is not a substitute for health insurance.

We do not charge enrollment fees, initiation fees, or hidden fees for participation in our DPC membership program. Membership pricing, included services, and terms are governed by a separate written membership agreement and may be updated from time to time.

Nothing on this Site guarantees insurance coverage, reimbursement, service availability, or medical outcomes.

Insurance Disclaimer

Gloria Gates CARE does not guarantee insurance coverage or payment for medical services. Insurance benefits, coverage determinations, and patient responsibility are governed solely by the terms of the patient’s individual insurance plan.

Patients are responsible for understanding their insurance coverage and any amounts not covered by insurance.

Forms & User Submissions

When you submit information through contact forms or other Site features:

• You represent that the information provided is accurate and lawful

• You acknowledge that submissions do not establish a provider–patient relationship

• You authorize us to use the information to respond to inquiries and provide services, consistent with our Privacy Policy
