Primary Care Provider Visits: Providers can coordinate your care with a wide range of medical professionals.

Pharmacy:

• Onsite clinical pharmacists are available for personalized consultation on medication management, medication/device demonstration, and patient assistance programs.
• Medications available onsite for the most common, short-term illnesses. 
• Medication formulary covers many of the most commonly prescribed GENERIC medications provided to you by one of our participating independent pharmacy partners.

Full-Service Lab: In-house sample collection adn testing, providing a wide range of tests with fast results and minimal wait times. 

Screenings:

• Prostate Cancer
• Heart and Lung
• Liver and Kidney 
• Cholesterol 
• Diabetes
• Mammograms
• Bone Density 
• In-Home Sleep Studies

Diagnostic Testing: Includes imaging, cardiology, and pulmonology tests as ordered by your provider.

• Imaging (X-Rays, CT Scan, MRIs, Ultrasound)
• Cardiology (Stress tests, Echocardiogram, Doppler studies, EKGs)
• Pulmonology (Lung Function and Spirometry)

Therapies:

• Onsite physical therapy
• Behavioral and mental health counseling
• Dietitians: education and advice on nutrition and healthy eating

DME – Durable Medical Equipment: Equipment to help people with injuries, such as walking boots, splints, braces, crutches, and CPAPs

Membership Card:

• Once you complete your Gloria Gates CARE Direct sign-up, you’ll receive your membership card via email.
• Save the PDF attachment to your files or camera roll.
• Show your membership card before your appointments with us and with any of our local partner providers.

While your subscription provides comprehensive primary care, it’s important to understand what’s not covered. Here’s a list of services and items that are not included in your subscription.

Emergency Room Visits: Your subscription does not cover emergency room visits. For life-threatening emergencies, always call 911 or go to your nearest emergency care center.

Urgent Care Centers: Visits to standalone urgent care facilities are not covered.

Specialized Medications:

• Experimental drugs
• Non-generic medications for acute conditions
• Brand name or injectable chronic disease medications (i.e., insulin therapy)
• Specialty drugs (such as those for cancer treatment)
• Most vaccines

Major Medical Procedures: Surgeries, inpatient/outpatient hospital stays, cancer treatments, dialysis, and many other specialized services.

Specialized Care:

• Specialized visits
• Inpatient treatment or admissions

Advanced Diagnostic Procedures:

• Genetic testing
• Biopsies
• Neurology testing, EEG, EMG

Dental and Vision Care: No dental or vision services are included.

Alternative Therapies: Services like acupuncture, chiropractic care, or massage therapy are not included.

Home Health Care: Nursing or medical care provided in your home is not part of this subscription.

Transportation: Provided by us to and from your appointments is not included.

Care Advocacy: Services are not included in Gloria Gates CARE Direct membership.
